Facility Use Request Form

Please fill out this form to request the use of Access Church facilities.  The completion of this form does not necessarily mean your requested function and/or date is approved.  A staff member will be in contact with you.

Facility Purpose Statement

Access Church facilities were provided by God’s benevolence and the sacrificial generosity of its members. Although the facilities are not generally open for outside use, Access Church makes its facilities available to approved people(s) and/or organizations on a case-by-case basis as a witness to its faith, in a spirit of Christian charity, and as a means of demonstrating the Gospel of Jesus Christ in practice.

Access Church facilities may not be used for activities that contradict, or are inconsistent with, our Tenets of Faith, as summarized in Constitution, Article IV. This restricted facility use policy is necessary for two reasons. First, Access Church may not in good conscience materially cooperate in activities or promote beliefs that are contrary to its faith (2 Cor. 6:14; 1 Thess. 5:22). Second, Access Church must present a consistent public witness to the community through its stewardship of its property. Allowing facilities to be used in a way that contradicts our Tenets of Faith would have a severe, negative impact on the message that the church strives to promote and could cause confusion and scandal to church and the community. Therefore, only events that are consistent with our Tenets of Faith, as determined by our church staff, shall be permitted.

Regarding times, please include the event time, the time you would like access to the facility, and the anticipated time you will be leaving the facility. For example, a wedding might look like: Ceremony from 2-3pm, access at 9am, leaving by 4pm.

Rental Spaces and Cost

All persons/organizations who are not Shareholders of Access Church are required to pay a $250 refundable security deposit, should there be no damages incurred. In addition, any non-affiliated persons/organizations are also required to pay a minimum $75 cleaning fee, which may be increased upon the discretion of church staff after the event is over.

*Auditorium requires at least one tech person at $20/hr. Additional people may be required based on usage. Camera platforms at the back of the auditorium and instruments on stage may NOT be moved.
**Usage of kitchen does NOT include any paper or perishable items (paper plates, plastic forks, coffee, bottled water, etc)
***Cost is per classroom (up to 3 available)

•Shareholders of Access Church are eligible for a minimum 50% discount on usage.
•For non-profits, the rental fees are waived.
•Cleaning fees may be waived at the discretion of church staff for shareholders and non-profits.

All fees charged are for the express purpose of recovering costs associated with ministering to users by making our church facility available and not for the intent of making a profit.

Choose as many as you need. Please note that each additional space constitutes a higher cost.

Facility Use Guidelines

1. Users may only use those areas of the facilities that have been reserved.
2. Alcohol: No alcohol may be served on church property without approval of the Lead Pastor and at least one Elder. If alcohol is served to the public, a liquor license (or similar) is required and no "open/unsupervised" serving is permitted at all.
3. Smoking is prohibited anywhere indoors.
4. Gender-specific changing areas, restrooms, and showers are to be used by members of the designated biological sex only.
5. Church equipment must be returned to original placement, unless arranged otherwise prior to the event.
6. All lights must be turned off and doors locked upon departure.
7. Abusive or foul language, violent behavior, or any illegal activities are strictly prohibited on church premises. Any person exhibiting such behavior will be required to leave the premises.
8. Depending on usage, the User must obtain liability insurance coverage in the amount of $1,000,000, naming Access Church as an additional insured. The User must also sign a “Facility Use and an Indemnity and Hold Harmless Agreement" upon acceptance of facility usage.
